APD provides stewardship for all Department of the Army forms and manages the life cycle of Army forms (cradle to grave). The Forms Management Branch reviews, develops, and creates forms that are used Army-wide. The Branch conducts a “forms review” on all administrative publications for forms implications.
The new fillable PDF forms have replaced both the Lotus Forms in XFDL format and the non-fillable PDF forms.
To create, revise or cancel a DA form, the customer must e-mail a completed DD Form 67 (Forms Processing Action Request) to APD Forms Management Branch
