NEWS: MILPER MESSAGE NUMBER 15-086

CHANGES TO EVALUATION SUBMISSION PROCEDURES FOR DA FORM 2166-8, NCO EVALUATION REPORTS (NCOERs) AND DA FORM 67-9, OFFICER EVALUATION REPORTS (OERs).

The purpose of the message is to provide guidance and procedures concerning submission methods for DA Form 2166-8 and DA Form 67-9 as well as provide timelines and dates indicating the discontinued use of the MYFORMs portal on the Army Knowledge Online (AKO) webpage. It also contains information concerning the forms conversion from IBM Lotus (XFDL) to fillable Adobe (PDF-F) form formats.

HQDA Form 5

For actions within the Headquarters Department of the Army, HQDA Form 5 must be used instead of DA Form 5.

Army Migrating to Adobe Fillable Forms

As of 1 October 2014, all DA forms are available in fillable Adobe PDF (PDF-F). The new forms replace the Lotus forms in XFDL format and non-fillable PDF forms. See instructions listed below on how to save completed XFDL forms in PDF or XPS Document Writer for your records. We recommend that users begin saving XFDL forms that are required historical documents with one of these methods.
